Aged Care Clinical Care Coordinator (CCC) - Permanent Full-Time
We are seeking an experienced Clinical Care Coordinator to join a residential aged care facility in Scottsdale Tasmania.
This is a permanent full-time position offering stability professional development and strong organisational support. Relocation assistance is provided for the right candidate.

Key Responsibilities
- Provide strong clinical and people leadership to support the delivery of safe high-quality nursing and care services for residents.
- Ensure appropriate nursing and care staffing levels are maintained in line with agreed workforce requirements and budget parameters.
- Oversee assessment care planning and review processes promoting active resident participation to support safe and effective ongoing care.
- Maintain accountability for accurate timely and compliant clinical documentation and reporting.
- Adopt a transparent and collaborative approach to clinical decision-making including open communication with residents and their families regarding care outcomes.
- Work closely with Clinical Governance and Education teams to support best practice and the consistent delivery of high-quality nursing care across the organisation.
Skills & Experience
- Strong clinical expertise with a minimum of 3 years recent experience in aged care nursing
- Proven capability in leading mentoring and managing multidisciplinary healthcare teams
- Comprehensive knowledge of aged care funding models National Aged Care Quality Standards and the National Quality Indicators Program
- Experience in identifying implementing and monitoring quality improvement initiatives
- A results-driven mindset with the ability to achieve performance outcomes against defined objectives
- Highly developed organisational prioritisation and time-management skills
