The Single Point of Entry (SPOE) Practitioner provides a clinically informed responsive and efficient entry pathway for referrals into Comcare Services.


About this role

The role ensures that referrals from Specialist Mental Health Services (SMHS) Hillmorton Hospital are triaged appropriately with clinical risk and service eligibility assessed in line with Comcares model of care.


What youll be doing

  • Lead the triage of SMHS referrals and ensure clients are matched to the right Comcare service with accuracy equity and timely decision making.
  • Apply strong clinical judgement to assess needs manage risk and escalate complex cases appropriately.
  • Work collaboratively with Comcares Triage Practitioner and SMHS teams to deliver a seamless coordinated SPOE intake process.
  • Build trusted clinical relationships represent Comcare at key meetings and support smooth transitions for clients entering our services.
  • Contribute to service improvement data reporting and culturally responsive practice aligned with Te Tiriti o Waitangi.


About you

To be successful in this role you will have:

  • Be a Registered Health Professional (e.g. RN Social Worker Psychologist Occupational Therapist) with at least three years experience in mental health and/or addiction services.
  • Bring proven expertise in clinical triage risk assessment and care coordination with the confidence to navigate complex or high-risk situations.
  • Demonstrate exceptional interpersonal and relationship building skills enabling effective collaboration across SMHS NGOs and multidisciplinary teams.
  • Be highly organised solutionsfocused and competent in electronic client systems (e.g. Recordbase) with a strong ability to problemsolve and prioritise in a dynamic environment.
  • Hold a solid understanding of Te Tiriti o Waitangi recovery principles and strengthsbased practice with a genuine commitment to equitable and culturally safe service delivery.
  • Experience in the mental health and disability sector would be an advantage.
  • Applicants must hold a current full clean drivers license.
