SUMMARY DESCRIPTION OF WORK

Professionals at this level have excellent communication skills with clients co-workers and community entities. This individual will have a high level of detail-orientation and a willingness work with clients and a variety of pay sources to ensure coverage for services. Specific project program or office responsibilities may occur at this level.

Compensation: between $19.74 and $23.54 per hour (wage is determined upon total years of relevant experience)

Work Schedule: Monday through Thursday 8:30 a.m. to 5:00 p.m. Friday 8:30 a.m. to 2:30 p.m.

MINIMUM QUALIFICATIONS

  • Bachelors Degree in Social Work or related field preferred.
  • Two years experience in Behavioral Health setting
  • Demonstrates a high level of social work skills practice and knowledge.
  • Valid Drivers license acceptable driving record.

Essential Duties and Responsibilities

  • Provide an assessment of clients presenting problem and barriers to recovery.
  • Assist the client in identifying the treatment options available to address the problems and make appropriate referrals to those programs.
  • Assist the client in problem solving any barriers involved preventing the pursuit of any treatment options including benefit management needs.
  • Provide information and referral to address client needs.
  • Provide Internal and external referrals to services as needed.
  • Check client benefits with insurance companies to ensure Medicaid/insurance coverage and that client understands coverage.
  • Help problem solve any insurance barriers that are found and identify and present possible solutions to these barriers.
  • Remain knowledgeable and innovative with community resources to help navigate a client with identified needs
  • Help engage and motivate clients into treatment/programming by utilizing a motivational interviewing skillset
  • Assist the client in problem solving barriers for community resources
  • Establish and maintain collaborative referral systems with both internal programming and community partners/referral sources.
  • Develop and run groups with members of the community who are in community placement such as hospitals and correctional facilities and help education and connect them to appropriate resources and supports.
  • Provide case management to clients who are transitioning yet facing barriers into programming until program enrollment is possible.
  • Enter collected data into computer database schedule intake/follow-up appointments and update information as needed.
  • Complete and process Release of Information forms to obtain necessary clinical and other data significant to a clients treatment.
  • Maintain a system of records and reports necessary for statistical documentation of referrals and the tracking of clients from initial inquiry through admission to agency or advisement of appropriate external resources.
  • Maintain all reports required of the position.
  • Display a comfort level in working with stressful individual/family dynamics.
  • Displays a courteous and caring attitude at all times to the clientele volunteers and visitors of the agency.
  • Cooperates and collaborates with program area staff volunteers and other Agency staff.
  • Is dependable and punctual regarding scheduling and attendance.
  • Abides by all specific program and Agency procedures policies and requirements.
  • Develops personal and program related skills through participation in internal and externaltraining opportunities including printed material and audio and/or visual media.
  • Creates maintains and shares as appropriate a dynamic self-care plan.
  • Strives to make connections between the agency and the larger community whenever possible in order to contribute to the agencys ongoing fundraising and friend-raising efforts.
  • Performs other program related duties as assigned.
