What Youll Do

- Manage chronic conditions with skill and empathy.
- Collaborate across clinical disciplines (primary psych palliative neuro) for whole-person care.
- Build trusted relationships with residents caregivers and community partners.
- Maintain thorough documentation and consistent communication.
What Youll Bring
- Active unencumbered NP or PA license in the state you will be practicing.
- Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
- Compassionate patient-centered approach and strong clinical judgment.
- Valid drivers license and reliable transportation.
