About us:
Catholic Community Services Foundational Community Support (FCS) Program provides Supportive Housing and Supportive Employment Services under the WA State Medicaid Transformation Project. We are looking for a compassionate and dedicated Housing Case Manager to join our team and make a positive impact every day.
Position overview:
As a Housing Case Manager you will provide outreach ongoing case management and individualize stability plan to increase residents stability and well-being at the Thea Bowen apartment.
Benefits & Compensation
This is a Full-time 40-hour position starting at $26.22 - $29.17/hr (D.O.E) witha competitive benefits package:

Responsibilities
Key responsibilities:
- Provide compassionate trauma-informed and responsive supportive services to a caseload of about 30 residents to include both FCS and non-FCS individuals.
- Assist clients with connecting to or applying for financial resources that will assist with housing stability such as HEN/ABD Social Security Emergency Assistance or vouchers/donations for furniture homeware and other basic essential needs.
- Coordinate and link clients to services including medical financial behavioral health appointments and Care Giving services
- Provide supports to assist the individual in communicating with the landlord and/or property manager regarding their housing needs. Address barriers to housing stability with the client.
- Document case management interactions via DAP-specific case note formats ensuring that case management sessions directly tie to goal-progression and meet billable format
- Provide crisis de-escalation and risk assessments for residents as needed.
Qualifications
Minimum qualification:
- A Bachelors Degree in a human/social services field; may also be an Associates degree in a relevant field with one year of field experience. May also be a Certified Peer Counselor with one year of field experience.
- 1 year of experience working with individuals experiencing homelessness or housing transitions.
- Active Agency Affiliated Counselors license (HIV/AIDS training required) or ability to obtain a credential within 60 days of hire.
- Demonstrable case management experience including problem-solving/issue assessment skills intervention planning/implementation skills and/or crisis intervention skills.
- Familiarity with principles of motivational interviewing trauma informed care and harm reduction
- Criminal history background checks are required prior to employment.
Preferred:
- 1 year experience working with individuals with behavioral health disorders.
- Experience working within the emergency shelter system and/or resident setting.
- Spanish language proficiency.
