Schedule: MondayFriday 9:00 AM 5:00 PM
Pay Rate: $52$60 per hour
Position Summary
Navitas Healthcare LLC is seeking a Licensed Master Social Worker (LMSW) with hospital-based experience to provide inpatient social work services in an acute care environment. The LMSW will support patients across medical surgical and rehabilitation units delivering psychosocial assessments coordinating interdisciplinary care and facilitating safe and effective discharge planning.
Key Responsibilities
Conduct comprehensive psychosocial assessments for inpatient populations
Provide therapeutic interventions and emotional support to patients and families
Collaborate with interdisciplinary healthcare teams to support patient care goals
Coordinate discharge planning and post-acute services including community resources
Advocate for patients social emotional and environmental needs
Maintain accurate timely documentation in the electronic medical record
Ensure compliance with hospital policies regulatory standards and confidentiality requirements
Required Qualifications
Minimum 1 year of experience as a Licensed Master Social Worker
Experience in inpatient medicine surgery or rehabilitation settings
Prior hospital-based social work experience strongly preferred
Education
Masters Degree in Social Work (MSW) required
Licensure & Certification
Active Licensed Master Social Worker (LMSW) license issued by NYSED
