Case Manager RN Clinical Oversight & Patient Care

Schedule & Compensation
- MondayFriday 8:00 AM5:00 PM
- Some after-hours and weekend responsibilities
- Salary range: $70000$80000
- Weekly pay
How You Will Make a Difference
In this role you will:
- Oversee patient cases from admission through discharge ensuring care plans are clinically sound compliant and individualized
- Monitor quality of care delivery including documentation outcomes and adherence to clinical standards
- Maintain active involvement in bedside nursing completing select patient visits to stay clinically connected and support care continuity
- Serve as a clinical resource and leader for nurses and caregivers offering guidance education and support
- Facilitate weekly case management meetings to review patient progress identify risks and improve care strategies
- Collaborate closely with leadership referral sources and interdisciplinary partners to ensure seamless communication and optimal patient outcomes
- Identify opportunities for process improvement and contribute to the ongoing growth of clinical excellence within the organization
This position is well-suited for an RN who wants to step into leadership without stepping away from patient care.
Whats Required to Join Our Team
- Active Registered Nurse (RN) license
- Minimum 2 years of nursing experience
- Strong clinical judgment with a detail-oriented approach to patient care and documentation
- Excellent interpersonal and communication skillsyou will interact with patients families clinicians and referral partners daily
- A collaborative mindset with an interest in mentorship quality improvement and care coordination
