We are currently recruiting a Medical Social Worker for a leading nursing home in Limerick Ireland.
Key Responsibilities
Demonstrate flexibility commitment and enthusiasm in delivering person-centred care to achieve the best possible outcomes for every patient.
Provide a comprehensive medical social work and counselling service to patients and their families across the care continuum.
Develop and maintain effective collaborative working relationships within the multidisciplinary team to support holistic patient-centred care.
Assess psychosocial needs and develop robust individualised social work care plans in partnership with patients families and the multidisciplinary team to support safe and effective discharge and community transition.
Provide counselling emotional and practical support to assist patients and families in coping with the psychosocial impact of illness.
Act as a key resource to the multidisciplinary team by providing expertise on psychosocial environmental and systemic factors affecting patients wellbeing and recovery.
Participate in peer supervision and when directed by the Principal Social Worker lead or contribute to peer supervision sessions in line with professional standards.
Engage in safe and supported lone working practices and participate in buddy systems to promote best practice and staff wellbeing.
Liaise and consult with internal and external agencies including community and voluntary services to ensure coordinated care and continuity of support.
Deliver social work services in compliance with relevant legislation CORU Code of Ethics GDPR FOI requirements and all organisational policies procedures and best practice guidelines undertaking additional duties as required by service needs.
Requirements
Must be registered on the Social Workers Register maintained by the Social Workers Registration Board at CORU.
Must maintain current live annual registration with CORU for the duration of employment.
Demonstrate the knowledge skills and competence required to effectively carry out the duties of the role.
Provide evidence of ongoing Continuing Professional Development (CPD) relevant to the area of specialisation.
Have a minimum of one years relevant post-qualification experience
