Introduction:
Beaumont Hospital is the National Kidney Transplant Centre carrying out approximately 150 170 kidney transplants per year. It is hoped to increase this number to 50 transplants per million of the population (200 250) per annum over the next few years.


Job Description
Renal Transplant Co-ordinator will be based in Beaumont Hospital to support the National Kidney Transplant Deceased and Live Kidney Donor Programmes.
To co-ordinate the renal transplant process and to work with the multidisciplinary team to ensure that all suitable potential kidney recipients and their families are supported through the transplantation process.
The combined role of Renal Transplant Co-ordinator involves both Live donation and deceased recipient transplant coordination responsibilities.
The postholder(s) will be expected to comply with the requirements of Statutory Instrument S.I. No. 325 of 2012 European Union (Quality And Safety Of Human Organs Intended For Transplantation) Regulations 2012 and subsequent amendments

Responsibilities


Live Donor Co-ordination
1. To co-ordinate the Live Kidney Donor Programme in Beaumont Hospital.
2. Provide a resource base on living kidney donor transplantation.
3. To co-ordinate the live kidney donor surgery and subsequent transplantation offering information and support to both the donor and recipient pre and post-operatively with the co-ordination of pre-operative tests and surgical interventions.
4. Attend the Live Donor Clinic to facilitate the donor workup recipient evaluation prior to transplant and post donation follow up visits.
5. Maintain accurate documentation on all aspects of living kidney donor transplantation.
6. Abide by statutory rules and legal requirements relating to practice.
7. Ongoing communication with the regional centres referring cases for live donation
8. Co-ordinate Paired Kidney Exchange Programme and liaising with UK for all renal centres nationally.
9. Liaising with NHISSOT on an ongoing basis to facilitate the immunological screening of all potential donors and recipients
10. Prepare facilitate and attend Consensus Meeting for Live Kidney Donation.

Renal Recipient Co-ordination
1. To co-ordinate the renal transplant programme in collaboration with the multi-disciplinary team.
2. To provide on-call cover responsible for receiving the donor offer liaising with renal surgeons and co-ordinating the renal transplant following deceased organ donation.
3. Assist with transplant recipient selection with the on-call transplant surgeon and the NHISSOT
4. Contact potential recipients and co-ordinate the transplant operation with the relevant departments in the hospital
5. The management of pre-transplant assessment programme and provide support for transplant patients on the waiting list.
6. Attend the pre-transplant assessment clinic meeting with and reassuring all patients being assessed for transplantation.
7. Monitoring of all patients on the waiting list for renal transplantation & liaising with NHISSOT
8. To co-ordinate with paediatric transplant centres .
9. Ongoing communication with Dialysis Centres throughout the country regarding patients on the transplant waiting list
10. Organise and attend monthly listing meeting.
11. Maintain accurate records of donor and recipients post renal transplant to ensure traceability.


Clinical Leadership
1. Plan and develop in collaboration with Transplant Surgeons Nephrologists and the Renal Care Team assessment and follow up of potential live kidney donors and recipients.
2. To be responsible for the initial referral of potential living kidney donors and recipients in order to assess the altruistic aspect of donation.
3. Act as a contact point for donors recipients and their families pre and in the initial post transplant phase.
4. In conjunction with donors/recipients clinician refer as necessary to appropriate service.
5. Establish a single pathway to facilitate recruitment and assessment of potential living donors.
6. Arrange record and report all the required investigations prior to living donor transplantation in accordance with national standards and ensure excellent communication with all parties whilst fulfilling the requirements for patient confidentiality.
7. Provide liaison with all relevant departments and members of the multi-professional team in order to deliver a planned seamless service for donors and recipients throughout.
8. Establish documentation which accurately reflects the donor assessment process and provides an accurate copy for the patients chart.


Audit/Research
1. Collate and present annual audit data and facilitate the implementation of recommendations for change.
2. Participate in pertinent multi professional clinical and academic research.
3. Participate in the provision of accurate collection of data on Live Kidney Donation for Organ Donation and Transplant Ireland.
4. Contribute to regular audit of the Living donor programme and to help introduce new strategies as appropriate.
5. Participate in benchmarking performance and activity with other major renal transplant centres in order to share learning and develop expertise.


Managerial Responsibilities
1. Provide liaison with and support for regional renal centres to facilitate living kidney donor transplantation.
2 .Investigate complaints/incidents and accidents and formulate reports as required and to resolve any underlying problems in accordance with Hospital policy.
3. Maintain all documentation in relation to patient care in accordance with guidelines for record keeping.


Education/Publicity Responsibilities
1. To be responsible for and actively continue own professional development with regard to advanced nursing practice.
2. In conjunction with Renal Care Team develop and co-ordinate a transplant recipient information/education including leaflets videos and seminars working closely with members of the multi disciplinary team.
3. Co-ordinate Live Donor/Recipient Unit Open Days.
4. Provide education training and support for all professional colleagues including undergraduate and post-graduate teaching programmes and in-service training initiatives.

Qualifications

Selection Criteria:
Selection criteria outline the qualifications skills knowledge and/or experience that the successful candidate would need to demonstrate for successful discharge of the responsibilities of the post.
Applications will be assessed on the basis of how well candidates satisfy these criteria.


Mandatory:

1. Statutory Registration Professional Qualifications Experience etc
(a) Eligible applicants will be those who on the closing date for the competition:
(i) Are registered in the relevant division of the Register of Nurses & Midwives
maintained by the Nursing and Midwifery Board of Ireland NMBI (Bord
Altranais agus Cnáimhseachais na hÉireann) or entitled to be so registered.

(ii) Have at least 5 years post registration experience (or an aggregrate of 5 years
fulltime post registration experience) of which 2 years must be in the speciality
or related area.

(iii) Have the clinical managerial and administrative capacity to properly discharge
the functions of the role.

(iv) Candidates must demonstrate evidence of continuous professional development.

(b) Candidates must possess the requisite knowledge and ability including a high standard of
suitability and clinical managerial and administrative capacity to properly discharge the
functions of the role.


2. Annual registration
(i) On appointment practitioners must maintain live annual registration on the relevant
division of the Register of Nurses and Midwives maintained by the Nursing and
Midwifery Board of Ireland (Bord Altranais agus Cnáimhseachais na hÉireann).

(ii) Confirm annual registration with NMBI to the HSE by way of the annual Patient
Safety Assurance Certificate (PSAC).

3. Health
Candidates for and any person holding the office must be fully competent and capable of
undertaking the duties attached to the office and be in a state of health such as would indicate a
reasonable prospect of ability to render regular and efficient service.


4. Character
Candidates for and any person holding the office must be of good character.

  • A management course
  • To have a recognised qualification in nephrology urology or transplant nursing or the willingness to undertake same
