Job Summary
Hospice Social Worker
Salary: $60-67k/yr BOE
Schedule: M-F 8:00AM-5:00PM (on call)
Location: Dayton Ohio and surrounding areas
Requirements of the Hospice MSW role:
Licensed Social Worker (LSW) in the state of practice
Bachelors Degree from a school of Social Work accredited by the Council of Social Work Education Masters degree preferred.
One year of social work experience in a healthcare setting preferred
Current Drivers License vehicle insurance and access to dependable transportation.
About the Hospice MSW role:
Provide medical social services to the patient and/or family according to the Plan of Care.
Conduct a psychosocial assessment as part of the comprehensive assessment within 5 days of admission to the hospice program including emotional social financial and environmental resources and identify appropriate psychosocial problems interventions and goals for the patients care.
Participate in IDG meetings and activities and the development of the patient plan of care.
Observe record and report changes in the patients emotional and social factors that are being affected by the patients illness and his/her need for care and response to treatment.
Maintain and submit written clinical records as deemed by Care 360 Hospice including the initial evaluation the care plan and daily notes.
Confirm weekly the scheduling of visits with the patients case managers to facilitate coordination of other staff visits.
Assess the patients and family members ability to cope with the patients passing.
Act as a consultant to the members of the health team assisting them in understanding the social emotional and environmental factors related to the patients health problems.
