Your Opportunity

Looking to make a real impact with young people Join us as a Case Manager and help high school students re-engage with learning and life! Youll work one-on-one with youth aged 1521 (or 25 for young parents) creating personalised learning plans and providing the support they need to thrive. Your key responsibilities will be to:

  • Respond to referrals of clients to the service from internal and external support services and conduct over-the-phone and formal face-to-face assessments of suitability for support
  • Register and induct clients into the service including the property facilities financials and regulations
  • Provide ongoing case management sessions (formal and informal) with clients and review progression against case plans and provide informal counselling as required
  • Assist clients in the process of transition out of the service into independence or other services
  • Development of an agreed individual learning plan (ILP) in conjunction with (if needed) an agreed case plan including goals and outcomes
  • Develop strong internal and external relationships with clients staff and key stakeholders to contribute to the effective functioning of the service and improved outcomes

Requirements for Success

  • Certificate IV or Diploma in Youth Work or Youth Justice Bachelor of Psychology or Bachelor of Social Work
  • Previous experience working in a case management approach within a education or employment focused environment
  • Previous training in providing a Child Safe Environment Reporting Abuse & Neglect and Education in Care
  • Current Working with Children Check
  • Current Drivers Licence
  • First Aid Certificate

Before starting work with us you will need to undertake a national police history check qualifications and referee checks and have a clearance to work with children/vulnerable people.
