The Advanced Heart Failure Program is seeking a dedicated and experienced Advanced Practice Provider to support the care of patients with complex heart failure and those undergoing evaluation for advanced therapies. The APP will provide highquality care in the outpatient setting as part of a collaborative multidisciplinary team.
Key Responsibilities
- Manage and monitor patients with advanced heart failure including comprehensive assessments and medication adjustments (e.g. GDMT diuretics).
- Order interpret and act on diagnostic studies including labs echocardiograms and other cardiac imaging.
- Educate patients and families on disease progression treatment options lifestyle modifications and care plans.
- Participate in heart transplant evaluation including gathering clinical data coordinating testing documenting findings and presenting in multidisciplinary transplant selection meetings.
- Support optimization of heart failure patients being considered for transplant.
- Collaborate with physicians nurses transplant coordinators and social work to ensure safe and efficient care.
- Contribute to quality improvement initiatives and program development.
Qualifications :
Qualifications
- Nurse Practitioner (NP) or Physician Assistant (PA) with active state licensure and national certification.
- BLS and ACLS required; DEA license as applicable.
- Preferred experience: cardiology advanced heart failure cardiac inpatient care or transplant evaluation.
- Strong communication skills clinical decisionmaking ability and comfort working in a fastpaced teambased environment.
Schedule
- Will involve outpatient responsibilities.
