Mobile Crisis Case Manager
Under the supervision of a Mental Health Program Director and Program Coordinator will assess individuals identify goals develop a person-centered plan for treatment implement treatment and link and coordinate community services all using the best practice strength-based person-centered model identified for the consumer. The position also includes responding to mental health crises through rapid response crisis stabilization assessment and connection to community resources in collaboration with crisis teams and first responders.

Qualifications and Experience:
Employment Qualifications:
Education: Possession of a Bachelors Degree in a Human Services-related field or Possession of a Bachelors Degree in Social Work.
Licensure: If the degree is in Social Work Michigan Licensed Bachelors Social Worker (LBSW) or Michigan Limited Licensed Bachelors Social Worker (LLBSW) with intent and ability to obtain an LBSW within two years of hire in this position.
Training in mental health interventions with children adults and families with mental health intellectual/developmental disabilities serious emotional disturbance or crisis intervention preferred.
Experience: Prior experience as a Case manager/Supports coordinator or in crisis response services preferred.
Other Requirements:
Possession of a Valid Michigan drivers license.
