Key Responsibilities
- Conduct thorough evaluations of clients in their home environments and develop individualized therapy programs with measurable short and longterm goals.
- Deliver treatments aligned with the physicians plan of care documenting progress reassessments and discharge summaries in a timely and accurate manner.
- Act as a liaison with physicians managed care providers aides and other members of the interdisciplinary team to ensure efficient coordination of care.
- Supervise a Physical Therapy Assistant (PTA): assess their ability to perform interventions ensure required cosignatures and monitor ongoing performance.
- Provide written homeexercise or care programs to patients/caregivers give education based on their needs and foster a respectful therapeutic relationship to promote goal achievement.
- Participate in discharge planning respond promptly to voicemails/messages update schedules in the system and document communications across the care team.
- Meet productivity standards consistently adhere to organizational competencies and standards of behavior and carry out additional duties or projects as assigned.
- Physical demands: lift/push/pull at least 50 lbs and stand for a minimum of 8 hours per day.
Required Qualifications
- Bachelors Degree in Physical Therapy or equivalent.
- Valid Physical Therapist license in the state.
- Basic Life Support (BLS) certification (Healthcare Provider level).
- Minimum of six (6) months of physical therapy experience.
- Must be able to travel to 4-6 patient homes per day within Monmouth County in New Jersey
- Proficiency with PC/computer use and documentation systems.
Preferred Qualifications
- Doctorate in Physical Therapy (DPT).
- Prior homecare experience.
