Key Responsibilities

Conduct thorough evaluations of clients in their home environments and develop individualized therapy programs with measurable short and longterm goals.
Deliver treatments aligned with the physicians plan of care documenting progress reassessments and discharge summaries in a timely and accurate manner.
Act as a liaison with physicians managed care providers aides and other members of the interdisciplinary team to ensure efficient coordination of care.
Supervise a Physical Therapy Assistant (PTA): assess their ability to perform interventions ensure required cosignatures and monitor ongoing performance.
Provide written homeexercise or care programs to patients/caregivers give education based on their needs and foster a respectful therapeutic relationship to promote goal achievement.
Participate in discharge planning respond promptly to voicemails/messages update schedules in the system and document communications across the care team.
Meet productivity standards consistently adhere to organizational competencies and standards of behavior and carry out additional duties or projects as assigned.
Physical demands: lift/push/pull at least 50 lbs and stand for a minimum of 8 hours per day.

Required Qualifications

Bachelors Degree in Physical Therapy or equivalent.
Valid Physical Therapist license in the state.
Basic Life Support (BLS) certification (Healthcare Provider level).
Minimum of six (6) months of physical therapy experience.
Must be able to travel to 4-6 patient homes per day within Monmouth County in New Jersey
Proficiency with PC/computer use and documentation systems.

Preferred Qualifications

Doctorate in Physical Therapy (DPT).
Prior homecare experience.
