Job Title:

Case Manager

Division/Program:

(Children/Youth Inpatient) Psychiatric Health Facility

Starting Compensation:

22.00 - 28.00 USD Per Hour

Working Location:

Bakersfield CA

Working Hours/Shift:

Monday - Friday 8 am to 4:30 pm

What you bring to SBHG:

High school diploma or equivalent required.
Associate Degree required
Bachelors degree in a social science or related field preferred.
(2) years experience in a mental health setting Preferred
Bilingual in Spanish preferred
Experience with case management services preferred.

How you will make a difference:

Participates in initial assessments and obtains outpatient treatment provider information options and resources.
Verifies follow-up appointments and collaborates with follow up treatment and step-down services.
Conducts groups to assist patients with transition to outpatient/community-based services.
Collaborates with PHF staff and caregivers to plan for and schedule discharge.
Provide community resources and psychoeducation. Submits discharge packets to referred agencies.
Documents all services provided in a clear and concise manner within the Electronic Medical Record within the timelines and framework established.
Transport clients to and from appointments hospital visits etc. as needed.
Develops and maintains relationships with external partners including DBH and DSS and outpatient providers.

Division/Program Overview:

24/7 inpatient involuntary program
16-bed facility for acute youth (ages 5-17)
Intensive treatment programs with individualized care plans
Available for contracting Central Valley counties
Moves clients through five phases: admission; engagement; learning; stabilization and linkage to resources and aftercare
