Your Opportunity

Wattle Houses Housing Support Services supports individuals and families who are homeless or at risk of homelessness to secure safe stable long-term accommodation while addressing the barriers that impact housing security. Based at Wattle House in Maddington this role sits within a multidisciplinary service hub delivering integrated housing and community support across Perths southeast.

As a Housing Case Manager you will work alongside individuals and families using a holistic person-centred and culturally responsive approach. You will support clients to build independence strengthen wellbeing and progress toward sustainable housing outcomes while navigating complex challenges such as housing instability financial stress family and domestic violence mental health alcohol and other drug use education and employment barriers.

Your Key Responsibilities

• Respond to referrals from internal and external stakeholders and conduct comprehensive suitability and needs assessments
• Undertake face-to-face client assessments using approved tools ensuring accurate and timely documentation
• Develop and implement holistic culturally responsive case plans that build tenancy stability independence and wellbeing
• Provide intensive ongoing case management through regular formal and informal client engagement
• Maintain professional boundaries while building client capacity resilience and informed decision-making
• Coordinate referrals and link clients with relevant supports including housing AOD FDV health education employment and community services
• Advocate for clients through collaboration with service providers preparation of support documentation and participation in case conferences.

Requirementsfor Success

• Significant experience in housing homelessness and/or human services with strong case management capability
• Certificate IV or Diploma in a relevant human or community services field with a minimum of three years sector experience
• Demonstrated knowledge of issues impacting people experiencing homelessness including FDV AOD mental health and unemployment
• Proven experience in assessment case planning advocacy referral and maintaining accurate timely case notes
• Strong networks and the ability to build effective relationships with housing providers community services and government agencies
• Ability to work effectively with diverse client cohorts including individuals families women and children with an understanding of family systems
• Excellent communication time management and organisational skills with confidence using Microsoft Office and case management systems.
• Full driver licence with no restrictions.
