Job Summary
Behavioral Crossroads LLC is seeking a part-time Mental Health Case Managerto join our partial care team! TheMental Health Case Managerhas primary responsibility for service coordination provision or arrangement of services needed personal advocacy and development review and updating of individual treatment and service plans. The position is in-office and schedule will be 4 days per week 5 hours per day.
Job Responsibilities include but are not limited to:
- Establish open lines of communication for individuals with mental or emotional issues
- Offer assistance to individuals with substance abuse/addiction issues and/or dual diagnosis
- Provide therapy to promote optimum wellness of emotional and mental health
- Implement and review treatment plans
- Collaborate with additional treatment providers and placement agencies
- Ensure all documentation is completed in a timely manner
- Conduct and document therapy sessions and weekly progress notes
- Complete treatment plans and comp assessments
To be considered for this position you must have at least one of the followingQualifications:
- Bachelors degree in the Human Services field or
- Associates degree AND two (2) years experience in providing Human Services or
- Five (5) years of Human Service experience or
- Possess a relevant professional credential such as Certified Psychiatric Rehabilitation Procedure (CPRP) Bachelors degree in Social Work Certified Rehabilitation Counselor (CRC) Certified Alcohol and Drug Counselor (CADC) Bachelors degree in rehabilitation or Bachelor of Science in Nursing.

Pay:$19.23 - $21.63 per hour
