This role is for a Home Health Physical Therapist Assistant (PTA) providing direct patient care in Portland OR and surrounding areas. The PTA will be responsible for delivering skilled physical therapy interventions to patients in their homes following established plans of care and working to improve patient mobility and function. The position requires a commitment to providing quality care and effective communication with the healthcare team.

Location:Portland Oregon United States
Responsibilities:
- Provide physical therapy treatments to patients in their homes according to the plan of care.
- Monitor and document patient progress adjusting interventions as needed.
- Educate patients and families on exercises and home programs.
- Communicate with the supervising physical therapist regarding patient status and any changes in condition.
- Maintain accurate and timely documentation of all patient encounters.
- Manage a caseload of patients optimizing visit schedules and travel routes.
- Achieve established productivity standards.

Required Skills & Certifications:
- Active Physical Therapist Assistant license.
- Basic Life Support (BLS) certification.
- Minimum of 1 year of skilled home health experience.
- Reliable transportation and valid drivers license.
- Car insurance.

Preferred Skills & Certifications:
- Experience with EPIC charting system.
- OASIS experience.

Special Considerations:
- Traveler must have their own transportation.
- The facility can call off UP TO one shift per pay period (every two weeks).
- Local candidates within a 50-75 mile radius are considered.
- Mileage reimbursement is provided at the IRS standard rate.

Scheduling:
- 8-hour day shifts.
- The facility cannot accommodate 4x10 or 3x12 schedules.
- No weekend requirements.
- On-call is not required.
