The Department
The Social Work Department operates in accordance with the philosophy and goals of The Alfred. A focus on quality of life and participation in care is a primary goal of the Department.
Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services which increase quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to home.

Position Summary
A fantastic opportunity exists for an experienced Social worker to work in the Trauma Service at The Alfred. You will have demonstrated prior experience in managing a complex clinical caseload and will have sound knowledge and skills in hospital discharge planning and providing complex interventions to facilitate discharge. An understanding of grief and loss counselling and psychosocial support is also essential. You will possess strong assessment case planning and have ewxperience in providing staff and student support. You will work within a strong multidisciplinary team and will be part of an organisation where professionalism innovation and the ability to find creative solutions to challenges is highly regarded. You will be required to work in any of the units within the Trauma stream as directed by the Trauma Team leader.
The role will involve providing comprehensive psychosocial assessments crisis intervention adjustment counselling therapy case planning family work community liaison networking and referral to external stakeholders and discharge planning.
The successful applicant will need to contribute to quality improvement and research activities and provide staff and student supervision.
Experience and Qualifications Required
Essential:
It is vital that the successful candidate has experience in working within an Electronic medical record. A knowledge of NDIS Aged careand VPTAS processesis essential for this role. Please ensure you address the Key Selection criteria in your application.
Please note: A current and valid Working with Childrens Check will be required to work within the core days in which the Cardiac Heart Failure and Transplant Team.
