What Youll Do (Essential Job Functions)

• Follow individual care plans and report any concerns to nursing or management.
• Administer medications and treatments with proper documentation per policy.
• Support daily living needs including:
  • Personal care
  • Meal preparation and dietary support
  • Household tasks and errands
  • Health appointments and community integration
  • Building natural supports and social relationships
• Promote health safety and a clean home environment.
• Remain calm and effective during challenging situations.
• Foster a positive respectful team dynamic with coworkers individuals and their families.
• Accurately document individual progress and changes.
• Attend all required trainings meetings and ongoing professional development.

What Youll Need (Minimum Requirements)

High school diploma or GED

Valid drivers license and reliable transportation

Ability to lift 25-50 lbs. (with assistance or lifting devices)

Physical ability to bend squat push/pull and kneel as needed

A compassionate and patient mindset
