We are seeking a Registered Nurse (RN) orEnrolled Nurse (EN) to join our team for a short-term project aimed atimproving vaccination uptake and coordinated care for high-risk patients.
About the Role
Working until June 2026 this role will focuson reviewing client records to identify patients who are overdue forvaccinations and other coordinated care needs. You will work closely with ourclinical team to support follow-up patient recall and improved preventativehealth outcomes.
Key Responsibilities Audit client records to identify high-risk patients overdue for vaccinations or care Coordinate follow-up appointments and patient recalls Support vaccination uptake and preventative health initiatives Work collaboratively with GPs nurses and Aboriginal Health Practitioners Maintain accurate clinical documentation and reporting
About You
Current AHPRA registration as a Registered Nurse or Enrolled Nurse Experience in primary health care community health or general practice Strong organisational skills and ability to review clinical records Excellent communication and patient engagement skills Commitment to culturally safe care for Aboriginal and Torres Strait Islander people
