The Department
The Social Work Department operates in accordance with the philosophy and goals of The Alfred. A focus on quality of life and participation in care is a primary goal of the Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services which increase quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to home.
The Position Summary
We are seeking aSocial Worker to join our Social Work teamatAlfred Health. Youwill have relevant hospital experience and be able to work within a strong multidisciplinary team. To be successful you will need tohave a good understanding of working and supporting patients with a chronic illness. Experience in counselling and adjustment to illness and hospitalisation is essential for this role. You will need toexhibit an understanding of social work theories and apply them to hospital practice. It will be essential to haveasound understanding of complex discharge planning within an acute hospital successful candidate will need excellent communication skills to collaborate with patients families other disciplines and community stakeholders. The successful candidate will need to possess strong advocacy skills for this role. The successful candidate will also need to have an understanding of Cystic Fibrosis.
The Alfred Health Cystic Fibrosis (CF) Service is an adult service for people living with CF in Victoria. The Social Worker is responsible for the provision of social work services to patients and their families/significant others primarily to the CF Unit. The role involves providing both outpatient and inpatient care. Participation in service programs such as the transfer of patients from the Royal Childrens Hospital to the Alfred is also an important aspect of the role. This role will also provide support to a designated clinical unit in Acute Social work to assist in-ward discharge planning/ counselling and psychosocial support through the provision of Social work clinical support. The Social worker will also be expected to be an active contributor to the team and Social work departmental activities such as quality improvement programs and development of service initiatives within the speciality area and priority allocation which may include priority triage for unallocated units and leave cover. Supervision of junior staff will also be required. Development and maintenance of community linkages are also important in this position. This role will be primarily based in the CF Unit with additional support provided to a designated Social Work clinical unit to constitute a full-time 1.0 EFT position.
