Salary: $91035.95 - $104336.27 (pro-rata) super

About the role

  • Delivering high quality evidence-based nursing care to people who are experiencing primary homelessness complex and co-occurring health and support needs in inner city Sydney.

  • Deliver comprehensive assessments clinical intervention care coordination and referral on an outreach basis.

  • Service provision is person-centred and aligned with the principles of trauma informed care and practice.

  • Collaborate with clients their professional cultural and social supports and our multidisciplinary team to address cultural holistic health and support needs using a strengths-based harm reduction approach.

  • Assist clients to access mainstream and specialist healthcare support and accommodation to improve their health and wellbeing outcomes.

  • Build and maintain strong partnerships and coordinate care with service providers and communities to strengthen client support networks.

  • Lead staff education contribute to clinical reviews and drive quality improvement across services.

About you:

  • Current registration with the Australian Health Professionals Regulation Agency (AHPRA) as a Registered Nurse.

  • At least 5 years experience delivering nursing assessment treatment and care coordination to people with complex health and support needs who are also experiencing homelessness.

  • Strong understanding of homelessness trauma-informed care and strengths-based frameworks and committed to clinical supervision and professional development.

  • Confident team member who drives service development education and quality improvement initiatives with excellent communication and interpersonal skills.

  • Tech-savvy and mobile comfortable with digital health records and holds a current NSW Drivers Licence.

There may be occasional early start times in this role.
