About the role
Delivering high quality evidence-based nursing care to people who are experiencing primary homelessness complex and co-occurring health and support needs in inner city Sydney.
Deliver comprehensive assessments clinical intervention care coordination and referral on an outreach basis.
Service provision is person-centred and aligned with the principles of trauma informed care and practice.
Collaborate with clients their professional cultural and social supports and our multidisciplinary team to address cultural holistic health and support needs using a strengths-based harm reduction approach.
Assist clients to access mainstream and specialist healthcare support and accommodation to improve their health and wellbeing outcomes.
Build and maintain strong partnerships and coordinate care with service providers and communities to strengthen client support networks.
Lead staff education contribute to clinical reviews and drive quality improvement across services.
About you:
Current registration with the Australian Health Professionals Regulation Agency (AHPRA) as a Registered Nurse.
At least 5 years experience delivering nursing assessment treatment and care coordination to people with complex health and support needs who are also experiencing homelessness.
Strong understanding of homelessness trauma-informed care and strengths-based frameworks and committed to clinical supervision and professional development.
Confident team member who drives service development education and quality improvement initiatives with excellent communication and interpersonal skills.
Tech-savvy and mobile comfortable with digital health records and holds a current NSW Drivers Licence.
There may be occasional early start times in this role.
