Seeking aCase Managerto join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Desert Cove Clinic.
Full-Time Employed: 40 hours/week
M-F 8:30a - 5:30p

Duties may include:
- Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals
- Completes assessments crisis plans and individualizedservice plans
- Works collaboratively with the clinical team to engage educate communicate and coordinate care with member their family behavioral health medical and dental providers community resources and others in ensuring that all services prescribed in the individualizedservice plan are implemented
- Conducts outreach and follow-up of services including but not limited to crisis interaction missed appointments and hospital discharges to ensure adequate resources are available and in place.
- Performs all case management functions associated with caseload including completing the assessment and service planning processes; including identifying the need for furtheror specialty evaluations
- Pursues best practice outcomes for person with mental illness including continuing education employment independent housing and community tenure

Qualifications- GED/High school diploma with a minimum of 1.5 years of behavioral health experience or
- An associatesdegree in the field of medical or behavioral or
- A bachelors degree (not in the field of behavioral health) and 1 year of behavioral health experience or
- A bachelors degree in the field of behavioral or
- A masters degree or higher in the field of behavioral health
- Computer Skills: Microsoft Word Excel Outlook electronic medical record NextGen preferred
- Valid AZ Driver have valid Arizona drivers license be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Healths driving policy
- Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment
- Interpersonal skills which allow for professional and positive relationships with co-workers members families and other community members
- Good communication skills evidenced by a non-judgmental approach effective listening and positive interactions
- Basic understanding of the DSM-V preferred
- Certified Case Manager strongly preferred
- Must pass background check TB test and other pre-employment screening.
