Job Summary
Registered Nurse RN - Case Management 40-hour position
WORK Inc. is seeking a Registered Nurse (RN) to join its nursing team to provide nurse case management and Medication Administration oversight to its community residential programs. The ideal candidate will provide nursing support to residential staff with a high degree of autonomy and be able to independently identify individuals needs and provide appropriate intervention. Supervision and support from Director of Health Services.
Responsibilities Include: Nurse case management collateral communication with caregivers conducting MAP audits MAP (Medication Administration Program) training medication administration support staff training and support discharge planning nursing assessments. Duties vary based on assigned site.
Hours: The position is 40hrs. five days per week with flexible hours and the only on-call responsibility will be PTO coverage. These are well established programs with excellent management retention and care. This position includes frequently going to residential homes and hospitals rehab centers etc.

Qualifications
Current MA license as an RN (Registered Nurse).
Will train and provide MAP (Medication Administration Program) certification for desired candidate.
