Responsibilities
We are looking for CNAs to join our skilled nursing teams at Community Subacute & Transitional Care Center! As a CNA on our team youll provide direct and indirect care to our diverse patient population under the direction of an RN/LVN. As an empathetic caregiver you will assist patients with daily living activities including but not limited to: personal care wound care feeding bathing and communicating with family.
When a patient needs ongoing care subacute care steps in. Community Subacute & Transitional Care Center is a 106-bed facility providing inpatient care for chronic subacute conditions such as coma or other complex illness that require 24-hour medical supervision.
Patients whose care requires ventilator respiratory wound IV hospice rehabilitation services and/or general nursing can receive treatment thats consistent with an acute care centers effectiveness while in a comfortable long-term care setting.
Qualifications
- High School Diploma High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
- Certified Nursing Assistant training to obtain certification required
- Incumbents hired into position prior to 05/14/2019 are grandfathered with their existing education degrees
- Experience in a skilled nursing facility respiratory subacute facility or acute hospital setting preferred
- CNA - State of California Certified Nursing Assistant Certification required
- BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
