Full-Time Direct Hire Outpatient Primary Care
About the Opportunity
A respected regional healthcare organization in western Connecticut is seeking Advanced Practice Providers (Nurse Practitioners or Physician Assistants)to join its growingInternal Medicine / Family Medicineoutpatient practices inSouthbury Naugatuck and Shelton. These arefull-time direct hireroles supporting busy primary care clinics focused on high-quality patient-centered care.
The APPs will work closely with supervising physicians and an integrated clinical team providing comprehensive primary care services including acute visits chronic disease management and preventive care for adult patients.
Position Highlights
- Three full-time employed APP positions (NP or PA) in primary care (IM/FM)
- Practice locations across Southbury Naugatuck and Shelton (site assignment based on candidate interest and practice needs)
- Outpatient-only schedule with emphasis on work-life balance
- Collaborative team-based environment with physician support and robust clinical staff
- Involvement in chronic disease management wellness visits and same-day/acute appointments

Requirements
- Licensed Nurse Practitioner or Physician Assistant (or eligibility)

- Strong interpersonal skills team orientation and commitment to patient-centered primary care
