Join University Home Care Inc as a Licensed Practical Nurse (LPN) and provide compassionate high-quality nursing care to our patients in St. Clair Shores MI. As an integral member of our healthcare team you will deliver skilled nursing services while ensuring patient comfort and safety in a supportive home care environment. You will be going to the client home 3 times a week. Its a total for 40 hrs a week.
Responsibilities
- Administer prescribed medications and treatments according to care plans.
- Monitor patients health by assessing vital signs and reporting changes to supervising healthcare professionals.
- Provide wound care injections and other nursing procedures as needed.
- Assist patients with activities of daily living and ensure their comfort and safety.
- Document patient progress and maintain accurate medical records.
- Collaborate with healthcare team members to develop and update individualized care plans.
- Educate patients and their families about health management and post-care instructions.
Requirements
- Current and valid LPN license in the state of Michigan.
- Minimum of 1 year experience in nursing preferably in home care or related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate patient-focused approach to care.
- Reliable transportation to visit patients at home.
