Job Summary

Description

We are searching for a Care Coordinator Assistant for the Cancer Center. Someone who will provide non-clinical administrative support to the Care Coordination process including but not restricted to timely discharge planning and utilization management.

Qualifications:

  • H.S. Diploma or GED required
  • Some college coursework preferred
  • 2 years experience in hospital care coordination/case management home health quality improvement or a related field required

Responsibilities:

  • Provides department-related and other appropriate communications to internal and external customers.
  • Provides assistance for utilization management processes to maintain compliance with federal state and managed care guidelines.
  • Provides assistance for transition/discharge planning and care coordination processes to transition plans and enhance patient/family satisfaction.
  • Maintains data integrity.
  • Provides professional efficient and courteous service to the customer while maintaining integrity in dealing with issues of a confidential nature.
  • Works effectively with peers nursing and medical staff and maintains a positive attitude. Responds to requests personally tactfully and courteously.
  • Obtains information required for response whenever possible and communicates pertinent information to staff customers and providers. Relays messages to appropriate individuals efficiently and in a timely manner.
  • Promotes a positive work environment and contributes to the overall team effort.
