DescriptionWe are searching for a Care Coordinator Assistant for the Cancer Center. Someone who will provide non-clinical administrative support to the Care Coordination process including but not restricted to timely discharge planning and utilization management.

Qualifications:
- H.S. Diploma or GED required
- Some college coursework preferred
- 2 years experience in hospital care coordination/case management home health quality improvement or a related field required
Responsibilities:
- Provides department-related and other appropriate communications to internal and external customers.
- Provides assistance for utilization management processes to maintain compliance with federal state and managed care guidelines.
- Provides assistance for transition/discharge planning and care coordination processes to transition plans and enhance patient/family satisfaction.
- Maintains data integrity.
- Provides professional efficient and courteous service to the customer while maintaining integrity in dealing with issues of a confidential nature.
- Works effectively with peers nursing and medical staff and maintains a positive attitude. Responds to requests personally tactfully and courteously.
- Obtains information required for response whenever possible and communicates pertinent information to staff customers and providers. Relays messages to appropriate individuals efficiently and in a timely manner.
- Promotes a positive work environment and contributes to the overall team effort.
