Job Description:
We are seeking a dedicated Physical Therapist for an outpatient role within a retirement community located just about a mile from Disneyland in Anaheim CA. This position offers a supportive environment working with independent living assisted living and memory care residents. The PT will provide high-quality therapeutic services focused on improving mobility function and quality of life for older adults.
Responsibilities:

Evaluate and develop individualized treatment plans for residents

Provide outpatient therapeutic interventions and ongoing assessments

Collaborate with interdisciplinary staff to support resident care goals

Maintain accurate and timely documentation

Deliver compassionate patient-centered care in a retirement community environment
Requirements:

Active California PT license

Prior outpatient or geriatric experience preferred
