Position Overview:
Seeking a skilled Physical Therapist to provide high-quality rehabilitative care to patients in accordance with state and federal regulations. The PT will evaluate patients develop individualized treatment and discharge plans and deliver therapy to enhance independence in daily living skills.
Responsibilities:
- Evaluate and treat patients per physician orders or referrals
- Develop implement and update individualized treatment and discharge plans
- Document services accurately and in a timely manner
- Supervise support staff according to practice acts and facility policies
- Collaborate with interdisciplinary care teams patients and families
- Ensure compliance with all safety regulatory and facility standards
Qualifications:
- Graduate of an accredited Physical Therapy program
- Active PT license in the state of California (or ability to obtain)
- Strong clinical documentation and communication skills
- Experience in rehab or post-acute care preferred
