Travel Physical Therapist (PT) Skilled Nursing / Rehab

Location: Salinas CA
Assignment Duration: Feb 23 2026 May 25 2026 (14 Weeks)
Shift: 8:00 AM 4:30 PM
Meal Break: 30 minutes
Schedule: Facility operates Sunday to Saturday (weekly roster assigned)
Guaranteed Hours: 35/week
Employment Type: Travel Contract

Role Summary

The Physical Therapist evaluates patients develops treatment plans and delivers rehabilitation interventions to improve mobility reduce pain and restore functional independence in a skilled nursing or post-acute care setting.

Key Responsibilities

Conduct patient evaluations and functional assessments
Develop individualized therapy plans per physician orders
Deliver manual therapy gait training strengthening and mobility programs
Track patient progress and modify treatment as needed
Complete timely EMR documentation and compliant therapy notes
Collaborate with interdisciplinary care teams
Educate patients and caregivers on recovery and safety strategies
Support discharge planning and care transitions

Qualifications

Active Physical Therapist license (state-specific in good standing)
Graduate of accredited PT program (DPT or equivalent)
Current BLS/CPR certification
Skilled nursing rehab or post-acute experience preferred
Strong documentation and caseload management skills
