Description
JOB SUMMARY: Evaluate and treats patients under order from the referring physician. Evaluation and treatment programs are developed through the use of a variety of therapeutic procedures and assistive devices intended to maximize functional potentials in all areas of a patients life. He/She functions in accordance with the State Practice Act.
PERFORMANCE DIMENSIONS AND TASKS
Essential Function
1. Review patient information and evaluate the impact of the information on the assessment and treatment process
2. Conduct and document a complete functional assessment of the patient in accordance with established guidelines and protocols.
3. Design and implement an individualized plan of care appropriate to the age and needs of the patient containing functional and measurable goals with input from both the patient and family.
4. Perform therapy treatment as ordered by the physician in a timely manner in accordance with established hospital guidelines.
5. Document progress in the patient medical record in accordance with established hospital and regulatory guidelines.
6. Meet established key management indicators for productivity levels of 9-12 patient visits per day.
7. Supervise instruct and train Rehabilitation Technicians in treatment procedures to be utilized while working with patients.
8. Communicate with physicians patients families other hospital personnel as needed regarding a patients progress problems and discharge plans.
9. Follow all approved documentation guidelines pertaining to the plan of care progress notes and discharge summaries.
10. Orient patients to treatment procedures appropriate to their age and needs.
a. Inform patients of an potential risk during any procedures
11. Establish and instruct patient family and/or caregiver in home programs and discharge programs.
12. Assist in the continuum of care process when a patient requires additional rehabilitation following discharge.
13. Attend and participate in required department meetings as well as other hospital committee meetings as assigned.
14. Participate in department quality improvement process.
15. Provide clinical supervision to Occupation Therapy students as requested
16. Access continuing education opportunities appropriate to discipline and responsibilities.
17. Act in compliance will all regulatory corporate and professional standards and practice guidelines.
18. Provide input to the Director of Physical Therapy on clinical issues and regarding clinical performance on COTAs rehabilitation technicians.
19. Performs other duties as assigned.
Requirements
QUALIFICATIONS
- Education: Bachelor of Science degree in Occupational Therapy
Skill Requirements:
- Ability to Assess patient and family needs and develop and implement a comprehensive Plan of Care.
- Knowledge of regulatory and accreditation standards/requirements.
- Knowledge of Medicare documentation standards.
- Recommended affiliation with the American Occupational Therapy Association (AOTA).
- Bilingual (English/Spanish) preferred
- Licensing Requirements: Current CA Occupational Therapist License
- Physical Demands: Constantly required to push/pull and lift light objects less than 50 lbs.
- Occasionally required to push/pull more than 300 lbs. and lifting in excess of 50 lbs.
- Required to perform simple motor skills such as standing walking writing and use of various types of therapeutic and office equipment
- Frequently required to remain in uncomfortable positions for long periods of time such as bending over equipment counters and tables and squatting.
- Required to walk stand and sit for long periods of time.
- Special Demands: Ability to work with others and to perform a variety of assigned duties.
Special Considerations: Will be dealing with patients of all ages to include; children/pediatrics adolescents adults and elderly geriatrics.
