Job Title: Registered Nurse (RN) Case Manager
Location: Syracuse NY
Contract Duration: 13 Weeks (Extendable)
Shift: Day Shift 9:00 AM 5:30 PM

A hospital in Syracuse NY is seeking an experienced RN Case Manager to support patient care coordination discharge planning and utilization management within an inpatient hospital setting. This role requires strong clinical judgment collaboration with interdisciplinary teams and expertise in acute care case management.
Key Responsibilities
Coordinate and manage patient care plans across the continuum of care
Facilitate discharge planning transitions of care and resource coordination
Collaborate with physicians nursing staff social workers and ancillary teams
Conduct utilization review and ensure compliance with payer and regulatory requirements
Identify and address barriers to timely and safe discharge
Document all case management activities accurately in the EMR
Participate in interdisciplinary rounds and care conferences
Advocate for patients and families to ensure optimal outcomes
Required Skills & Experience
Acute care hospital case management experience
Proficiency with EPIC Electronic Medical Records
Strong knowledge of discharge planning utilization review and care coordination
Excellent communication organizational and critical-thinking skills
Qualifications & Licensure
Active New York State RN License
AHA BLS certification (must be current or eligible for recertification)
Minimum 1.5 years of recent acute care experience (within the past 2 years)
Experience in hospitals of similar size complexity or trauma level required for travelers
