Job Summary

Shift: Monday - Friday (8am-5pm) Weekends: Some weekend coverage is required Call: Some call may be scheduled but only on an emergency basis Requirements: -Must have OASIS (Outcome and Assessment Information Set) Experience -Home Health and Hospice Experience required -Experience traveling from patient to patient for home visits -Must be able to manage and adjust to changes in schedule -Must have drivers license in good standing -BLS required -EMR system used is HealthWyse (Careficient) -Outpatient care -Facility does their own Background Check as well Come join our team today!!
