Position Summary
The Home Health Manager is responsible for leading and overseeing skilled home healthcare services for senior participants. This role ensures the delivery of high-quality participant-centered care across home settings nursing facilities and board and care environments. The manager provides clinical oversight supervises the home health team and collaborates with interdisciplinary partners to ensure individualized care plans are effectively implemented.
Key Responsibilities

Lead and manage the home health department ensuring adherence to established workflows policies and regulatory standards.

Recruit mentor train and evaluate home care staff to maintain a high-performing team.

Oversee nursing assessments and ensure active participation in interdisciplinary team (IDT) meetings.

Ensure accurate documentation and implementation of care plans by home health nurses.

Monitor and evaluate the quality of care provided in nursing homes and board and care facilities.

Maintain compliance with state and federal home health regulations and organizational best practices.
Required Qualifications

Active Registered Nurse (RN) license in the state of California.

Minimum of two (2) years of nursing experience in a home health setting preferably serving elderly or frail populations.

Strong clinical expertise in managing complex care needs of senior participants.

Bachelors degree in Nursing preferred; equivalent experience may be considered in lieu of formal education.
