Position Overview
The Behavioral Health Coordinator is responsible for supporting the integration and coordination of behavioral health services within Aspire Health programs. This position collaborates closely with clinical providers care teams and community partners to ensure timely access to behavioral health services and continuity of care for patients.
Essential Duties and Responsibilities

Coordinate behavioral health referrals intake processes and follow-up services

Serve as a primary point of contact between patients medical providers and behavioral health professionals

Assist with behavioral health screenings care planning and service coordination
-
Monitor patient engagement and progress ensuring appropriate documentation in electronic health records
-
Support care teams in addressing behavioral emotional and psychosocial needs
-
Collaborate with internal departments and external partners to improve service delivery
-
Ensure compliance with all confidentiality privacy and regulatory requirements
-
Participate in quality improvement and care coordination initiatives
Required Qualifications

Bachelors degree in Psychology Social Work Human Services or a related field
-
Minimum of one year of experience in behavioral health or care coordination
-
Strong understanding of behavioral health systems and community resources
-
Excellent communication organizational and documentation skills
-
Proficiency in electronic health record (EHR) systems
Preferred Qualifications
-
Masters degree in a behavioral healthrelated discipline
-
Active or eligible licensure (LCSW LMFT LPCC or equivalent)
-
Experience in integrated care or population health settings
-
Training in trauma-informed culturally responsive care
