Job Title: LPN / LVN - Long Term Care
Location: City of Utica NY

Duration: 3 months
Rate: $32.85/hr
Guaranteed Hours: 37.5 hours/week
Work Model: Onsite

Assignment Details

Setting: Long Term Care Facility
Unit Size: 40-bed unit
Patient Population:
- Multiple Sclerosis (MS)
- Orthopedics
- Dementia
- Memory Care
EMR System: PointClick Care

Schedule Requirements

Every other weekend required
Must float to all units within the same building
Local candidates accepted (local rate applies if within 75 miles)
75-mile radius rule applies for travel candidates

Key Responsibilities

Provide direct patient care in a long-term care environment
Administer medications and treatments as prescribed
Monitor patient conditions and report changes to nursing leadership
Accurately document care in PointClick Care EMR
Collaborate with RNs and interdisciplinary care teams
Support residents with daily living activities and clinical needs
Maintain compliance with facility policies and professional standards

Required Qualifications

Active New York State LPN/LVN License
BLS Certification
Minimum 1 year of LPN/LVN experience
Ability to float between units as needed
Experience working with geriatric and memory care populations preferred
