We are seeking a qualified and organized Resident Care Coordinator to oversee and coordinate resident care services in our Assisted Living community in Staten Island NY.
This position requires an active New York State LPN license. The Resident Care Coordinator is responsible for ensuring high-quality care delivery supervising care staff maintaining compliance and supporting residents clinical needs.
Essential Responsibilities
Coordinate and oversee daily resident care services within the assisted living program
Conduct resident assessments and monitor changes in condition
Ensure resident care plans are implemented and updated as needed
Supervise support and guide caregiving staff
Communicate effectively with residents families physicians and interdisciplinary team members
Maintain accurate clinical regulatory and resident documentation
Assist with admissions transfers and discharge coordination
Ensure compliance with New York State assisted living regulations and facility policies
Address care-related issues and support quality improvement initiatives
