Your responsibilities include comprehensive medication management therapeutic interchanges drug utilization review safe dispensing practices and direct patient counseling at discharge and in ambulatory clinics. Youll collaborate with physicians and care teams to optimize regimens identify medicationrelated problems monitor adverse events and contribute to pharmacovigilance initiatives.
