Join us in a permanent full-time role in the Ipswich region and make a meaningful difference every day. As a Recovery Support Worker in our Hospital to Home program you will deliver practical hands-on support to people as they transition from hospital care to independent living. Inthis role you will assist clients with daily activities support goal achievement build skills and encourage meaningful community participation. Youll maintain accurate records uphold high standards of safety and quality and work collaboratively with supervisors internal teams and external partners. By fostering a respectful inclusive team culture you will also contribute to continuous improvement and your own professional development. Thisrole is suited to someone who brings flexibility accountability and a genuine commitment to person-centred values-driven support.

Your Key Responsibilities:

  • Provide practical day-to-day client support including assistance with daily tasks recovery activities and progress toward identified goals.
  • Deliver person-centred recovery-focused support using evidence-based approaches grounded in compassion empowerment and mutual respect.
  • Support clients through key transitions including hospital discharge community re-engagement and securing sustainable housing.
  • Collaborate with internal and external stakeholders to build client capacity independence and community connection.
  • Assist supervisors with client assessments risk assessments and reporting to ensure safe high-quality service delivery.
  • Maintain accurate timely documentation and reports using organisational systems and mobile technologies.
  • Contribute positively to team culture and continuous improvement through supervision feedback processes and service enhancement initiatives.

You will have:

  • Diploma in a human service-related field and/or demonstrated previous experience working with people with mental illness
  • Ability to work autonomously and professionally within RFQs service philosophy and values
  • Demonstrated ability to effectively manage time own case load and KPIs including outcomes measures and safety planning
  • Excellent communication and interpersonal skills with the ability to effectively engage with clients colleagues stakeholders and service providers
  • A strong sense of professional boundaries and integrity
