All Heart Home Care is seeking skilled motivated Physical Therapists (PTs) and compassionate Physical Therapist Assistants (PTAs) to join our growing home health these roles you will provide personalized patient-centered therapy services in patients homes helping them improve mobility reduce pain restore function and regain independence following illness injury or surgery.
Service Locations
We are currently hiring for positions in the following areas:
- Nashville TN
- Murfreesboro TN
Key Responsibilities
For Physical Therapists (PTs):
- Evaluate patients physical conditions and develop individualized treatment plans
- Implement therapeutic exercises and interventions to improve strength balance and mobility
- Monitor patient progress and adjust treatment plans as needed
- Educate patients and caregivers on exercises safety and injury prevention
- Document evaluations treatments and outcomes accurately
- Collaborate with physicians nurses PTAs and other healthcare professionals
- Ensure compliance with all healthcare regulations and company policies
For Physical Therapist Assistants (PTAs):
- Implement treatment plans developed by the supervising Physical Therapist
- Assist patients with therapeutic exercises and functional mobility
- Monitor patient progress and report changes to the supervising PT
- Educate patients and caregivers on exercises safety and proper techniques
- Document treatments and patient responses accurately and in a timely manner
- Maintain a safe supportive environment during therapy sessions
- Comply with all healthcare regulations and company policies
Qualifications
PT Requirements:
- Active Physical Therapist (PT) license in the state of employment
- Minimum of 12 years of experience (home health experience preferred)
PTA Requirements:
- Active Physical Therapist Assistant (PTA) license in the state of employment
- Previous experience in home health or outpatient settings preferred
For Both Roles:
- Strong clinical and assessment skills
- Excellent communication and patient education abilities
- Ability to work independently and manage time effectively
- Compassionate patient-focused approach
- Current CPR certification
