Job Summary:
Join our dynamic team as a Community Health Specialist! In this pivotal role youll drive community engagement through health fairs targeted outreach and educational classes such as CPR and Mental Health First Aid. Youll support program activities conduct health screenings and connect participants with vital resources. Reporting to the Health Promotion Program Manager youll help transform lives and foster healthier communities.
Key Responsibilities:
- Deliver empowering health classes and up-to-date CPR and Mental Health First Aid training.
- Conduct health screenings and connect individuals to evidence-based programs.
- Facilitate referrals to local community resources.
- Maintain accurate records to track program participation and participant feedback.
- Cultivate in-depth knowledge of community health resources to better serve participants.
- Ensure seamless care transitions from hospital to home to community settings.
What Youll Bring:
- Exceptional communication and listening skills to engage with patients and families.
- Problem-solving proficiency and the ability to present information effectively.
- Proficiency in Microsoft Word and familiarity with computer-based systems.
Qualifications:
- High school diploma/GED and a passion for community health.
- Preferred experience in community settings and teamwork with diverse groups.
- Bilingual candidates (Spanish Korean or Chinese) are highly encouraged to apply.
- Must obtain CPR certification within 60 days of hire and hold a valid drivers license.

Salary Range: Minimum 1.00/hour - Maximum 27.00/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount salary discussions will occur during candidate screening calls before any subsequent compensation discussion is held between the candidate and any hiring authority.
