As a Nurse Case Manager you will support this mission by working within the Multipurpose Senior Services Program (MSSP) to deliver coordinated client-centered care in collaboration with MSSP Case Managers.

Nurse Case Managers perform case management duties both independently and as part of a multidisciplinary team. Responsibilities include conducting health assessments for frail elderly low-income program participants monitoring their health status making appropriate referrals and maintaining complete and accurate records.

This position offers a supportive work environment with clear clinical and program oversight from the Supervising Case Manager (SCM). Nursing assessments reassessments and Level of Care (LOC) documentation are reviewed to ensure quality compliance and adherence to MSSP and California Department of Aging (CDA) standards.

What you will typically be responsible for:
  • Under the clinical and programmatic oversight of the Supervising Case Manager (SCM) conducting nursing assessmentsincluding initial assessments reassessments and Level of Care (LOC) documentationand completing related documentation in accordance with MSSP site manual and California Department of Aging (CDA) requirements for completeness timeliness formatting and compliance.
  • Developing and updating comprehensive health care plans based on nursing assessments including LOC recertificationsin coordination with the interdisciplinary care team.
  • Working closely and collaboratively with MSSP Case Managers to ensure alignment between nursing assessments the Functional Needs Assessment Grid (FNAG) care plans service authorizations and participant goals.
  • Meeting all critical MSSP and site-specific documentation deadlines including but not limited to reassessments LOC updates and mandated visit documentations to ensure participant services billing and program compliance are not affected.
  • Arranging and/or ordering services as detailed in the care plan; monitoring client status and services by conducting monthly phone calls and quarterly home visits.
  • Participating in interdisciplinary care plan conferences.
  • Documenting all actions using program-specific forms and standards; maintaining case management charts resource files and confidentiality of client information.
  • Driving a vehicle to make monthly home visits and to participate on planning committees for health assessment activities.

Minimum Requirements for Application

This recruitment will produce two eligible lists. The restricted list will be for full-time permanent part-time or part-time City and Port of Oakland employees who maintain employment for the duration of the recruitment and pass all stages of the recruitment process. The open list will be for all other applicants who pass all stages of the recruitment process.

This is a public contact position. The City highly encourages all bilingual candidates to apply! Some positions may require bilingual skills in Spanish Cantonese or Mandarin. Under the authority of Civil Service Rules Section 5.03 if a position requires bilingual language skills only those candidates who have indicated they are bilingual in the required language(s) will be referred. Bilingual candidates will be required to successfully pass a language proficiency test if selected for hire.

Any combination of education and experience that is equivalent to the following minimum qualifications may be acceptable.

Education:

An Associates degree from an accredited college or university in nursing.


Experience:

One (1) year of nursing experience.

License or Certificate / Other Requirements:
Possession of current California Department of Consumer Affairs Board of Registered Nursing license in good standing.

Individuals who are appointed to this position will be required to maintain a valid California Drivers License throughout the tenure of employment OR demonstrate the ability to travel to various locations in a timely manner as required in the performance of duties.

Desirable Qualifications:

  • Valid California Drivers License
  • Bilingual preferably in Spanish or Chinese

The Recruitment Process:

The City of Oakland works with a Civil Service system. Applicants apply online and HR staff reviews their experience education and training listed on the application against the minimum qualifications listed for each position.

Candidates who demonstrate that they meet the minimum qualification requirements are then invited to participate in an assessment process (or testing) to measure their knowledge skills and abilities in job-related areas.

Candidates who do not demonstrate that they meet the minimum qualification requirements will have an opportunity to request a final review. Candidates must request a final review within 5 business days of being notified that they did not meet the minimum qualifications.

In the final review candidates may clarify information already provided on their application. No additional work history education or certifications will be accepted. Any decisions made during this review process will be final.

For this recruitment all qualified applicants will be sent a writing exercise (weighted 100%) which will then be evaluated and rated by subject matter experts. The tentative date that the writing exercise will be sent to you for completion is the week of January 19 2026.

Candidates who pass each stage in the process will be placed on an eligible list. Some candidates may receive extra points for example if you are a Veteran with an honorable discharge or if you are a current City of Oakland resident. If youre applying to a restricted posting and you are eligible as a current City employee you may also receive additional points for your City service. Extra points are only applied to passing scores so if you do not pass every element of the testing process you cannot use extra points to pass.
