Job Summary

We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality hospice and home health care to senior patients. This role focuses on patient comfort dignity and individualized care while working closely with families caregivers and interdisciplinary care teams.

Provide skilled nursing care to hospice and home health patients in their homes or senior living environments

Assess patient conditions and develop implement and evaluate individualized care plans

Administer medications treatments and wound care as ordered by physicians

Monitor patient symptoms pain levels and overall health status

Educate patients and families on care plans medications and symptom management

Collaborate with physicians social workers aides and other healthcare professionals

Maintain accurate and timely patient documentation and reports

Provide emotional support and compassionate care to patients and families

Follow all state federal and company policies and healthcare regulations

Requirements

Active Registered Nurse (RN) license

Strong assessment clinical and communication skills

Compassionate patient-centered approach to care

Ability to work independently in home-based settings

Valid drivers license and reliable transportation

CPR/BLS certification

Preferred Qualifications:

Hospice or palliative care experience
Experience working with geriatric patients
Knowledge of Medicare regulations and documentation standards

Work Environment & Physical Requirements:

Travel to patient homes and senior residences
Ability to lift reposition or assist patients as needed
Emotional resilience and strong interpersonal skills
