Job Summary:

The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaborating with an interdisciplinary team. Strong communication and collaboration skills are essential for working with patients providers and team members.

Required Skills & Certifications:

- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum 1 year of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license or pending license in the state of working
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work

Preferred Skills & Certifications:

- CA state license

Special Considerations:

- First-time travelers are welcome.

Scheduling:
- Start date: ASAP
- Weekend Requirement: 2 weekends out of 4 off; weekend 2 consecutive days (Fri-Sun).
- RTO Restrictions: No more than 7 days.
