Job Summary:
The Service Navigator Addictions is responsible for providing navigation support to clients with substance use issues in the Sunnybrook Emergency Department or an inpatient unit. The Addiction Service Navigator will support staff and patients by creating connections between Sunnybrooks Emergency Department inpatient teams the Rapid Access to Addictions Medicine Clinic (RAAM) and the community.
The person in this role will work closely with the Peer Service Navigator to support patients to access services they need to address challenges with their substance use and to improve their quality of life. The Addiction Service Navigator works in partnership with clients to develop a recovery plan and following the principles of recovery will also provide assistance with accessing community resources and support linking clients to appropriate services and developing a safe plan. The Addiction Service Navigator is also responsible for documenting interactions with clients and other relevant parties as required.
This role will be actively engaged with inter-professional colleagues from different areas within the hospital and community brought together to better meet the needs of people with substance use disorder.
What You Will Do
- Assists clients in the development and implementation of a care plan tailored to the clients needs and issues designed to improve quality of life. Identify and refer to resources specific to those needs and issues;
- Act as a liaison and client advocate among those involved with the clients care including family other caregivers medical and other multi-disciplinary providers.
- Provide supportive counselling or referral and linkages to health care programs or service providers.
- Provide crisis management/intervention and assist in the development of crisis plans for clients;
- Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles;
- Maintain strict confidentiality and follow all other required policies and procedures;
- Consistently interact with clients staff family members other community agencies and partners in a professional manner that reflects favorably on the organization.
- Attend and actively participate in all required training;
- Liaise collaborate and coordinate with other professionals service providers and partners;
- Participates in partnership meetings as required
- Participates in weekly team meetings individual supervision and regular agency administration meetings
- Takes responsibility for identifying areas for professional growth and training in conjunction with the Manager or Director
- Participates in ad-hoc committees within the agency providing input to improve the services provided by the agency
- Represents the Network at external workshops inter-agency initiatives staff training and other community-based activities
- Complete other duties from time to time as assigned by Program Director/Manager
What You Bring:- Completion of post-secondary education in social services/human services or equivalent
- Minimum 3 years of experience in providing support to adults with mental health and or substance use issues.
- An understanding of harm reduction and psychosocial rehabilitation principles the therapeutic relationship and an ability to come to an agreement with client on goals/priorities
- Crisis intervention skills with well-developed problem-solving abilities and interpersonal skills
- Group counselling related to mental health addictions and related issues
- Excellent Management of the privacy and confidentiality of all client information according to agency policy
- Comprehensive knowledge of mental health and housing services and networks in Toronto
- Accuraterecord keeping and reports time sheets expense claims and other required information in accordance with agency guidelines
- Good oral and written communication skills and computer literacy including MS word and Excel
- Ability to work effectively in a team environment
- Demonstrated effectiveness working with minimal supervision with proven reliability and trustworthiness
- Understanding of cultural competency and anti-oppression framework
- Proven experience with anti-racist and DEI practices.
- Ability to communicate in a different language an asset.
- A valid Ontario drivers license and a care is required.
