The Care Partner is the primary contact for participants and their supporters leading the delivery of the Support at Home program. The Care Partner plays a vital role in providing high quality relationship-based person-centred in-home care tailored to each individuals needs goals and preferences.
The Care Partner builds trust and rapport with their participants and supporters by working collaboratively to gain a deep insight into age-related health needs within the context of the participants life. This relationship-based support and planning underpins every care management activity completed by the Care Partner driving improved outcomes continuity of care and participant satisfaction.

This is a permanent full-time role based out of any of our Victorian Aged Care Services offices.
These offices include Bendigo Echuca Gisborne Shepparton and Swan Hilll.
This position has been classified under the Social Community Home Care and Disability Services Award as Level 4 with a starting annual salary of $88090 Superannuation Salary Packaging.
