Job Overview

Navitas Healthcare LLC is seeking a compassionate and experienced Social Worker for a full-time 14-week contract assignment in Camden Maine. This day-shift role offers a consistent weekday schedule and guaranteed 40 hours per week. The Social Worker will provide comprehensive psychosocial support advocacy and case management services to residents and their families within a healthcare setting.

Key Responsibilities

Conduct initial and ongoing psychosocial assessments to identify residents emotional social and environmental needs
Collaborate with interdisciplinary care teams to develop and implement individualized care plans
Coordinate complex discharge planning including community referrals home health services and durable medical equipment
Provide short-term counseling and crisis intervention to residents and families
Advocate for resident rights and assist with advance directives and power of attorney matters
Support residents and families with financial resource guidance including Medicaid and insurance navigation
Maintain accurate timely documentation in compliance with state and federal regulations

Qualifications

Licensure: Active Social Work license in the State of Maine (LSW LMSW or LCSW required)
Education: Bachelors degree in Social Work (BSW) required; Masters degree (MSW) preferred
Experience: 12 years of recent healthcare social work experience preferred (SNF Long-Term Care or Acute Care)
Skills:
- Strong knowledge of discharge planning Medicare/Medicaid and community resources
- Ability to work independently in a fast-paced environment
- Excellent communication and documentation skills
