Travel Social Worker

A well-established hospital located in Minnesota near Brookings South Dakota is seeking a contracted Social Worker to join its exceptional team.

Offering $50/hour

Social Worker Opportunity:

13-week contract
Schedule: 4 days a week 8 am - 4:30 pm
Reports to Hospital DON and Hospice Manager
Potential for a Permanent opportunity available
Medical Dental and Vision Coverage available

Travel Social Worker Responsibilities:

Be a core member of the Hospice team and provide support to patients and families emotional social and economic needs relating to the impact of a terminal illness
Plan the transition and transportation of patients from hospitals to a home or another facility arrange services and collaborate with doctors nurses and insurers
Plan and lead interdisciplinary meetings facilitate referrals for community resources and review critical access admissions and monitor patients with regularly scheduled admission visits

Travel Social Worker Qualifications:

Must have a Bachelors degree in Social Work
Must have a valid LSW license or the eligibility to obtain one
Previous experience in Hospice is a bonus
